ALL student-athletes are required to submit a Yearly Physical Form and an Updated Immunization Record.
In addition, a Permission to Treat Form must be completed as well.Please download the permission form HERE
and have a physician complete the NJSIAA Physical Form below. Hudson Catholic cannot accept any other physical form.
*ALL Students & Student-Athletes must have completed physical forms and supplemental forms before the start of the academic year.
